How to settle a claim for Endowment Insurance Plan?

Step 1:

Contact the insurance branch office on their toll-free number or website or email or mobile app.

Step 2:

Fill out a Claim Intimation form. The form can be found on the insurance company’s website. For example: LIC India claim intimation form.

If the insurance company does not have a Claim Intimation form, a Claim Intimation letter can be written. It should have the following information:

Policy Number

Name of Insured

Date of Admission in Hospital

Cause of Hospitalization

Hospital Address

Name of the Claimant

Step 3:

Submit the following documents to the insurance company:

Filled Claim Intimation form

Death Certificate (if applicable)

Copies of medical records, test reports, admission records of hospitals and labs (if applicable)

Critical Illness Claim Form filled by doctor (if applicable)

Original Policy Document

Deeds of Assignment

Claim Discharge Form

The Critical Illness Claim Form is a form that should be filled while claiming insurance for medical treatment.

Deeds of Assignment is a document which transfers the Life Insurance policy from one person to another (usually from the Life Assured to the Beneficiary).

Claim Discharge Form is given by insurance companies at the time of paying the sum assured to the claimant
