How to settle a claim for a Whole Life Insurance Plan?

    Contact the insurance branch office on their toll-free number or website or email or mobile app.

    Fill out a Claim Intimation form. The form can be found on the insurance company’s website. For example: LIC India claim intimation form.

    If the insurance company does not have a Claim Intimation form, a Claim Intimation letter can be written. It should have the following information:

    1. Policy Number
    2. Name of Insured
    3. Intimation of death (if applicable)
    4. Intimation of maturity (if applicable)
    5. Name of the Nominee/ Claimant

    1. Filled Claim Intimation form
    2. Copies of Death Certificate (if applicable)
    3. FIR
    4. Post Mortem Report in case of accidental death
    5. Certificate and medical records from doctor/ hospital if death due to illness
    6. Critical Illness Claim Form filled by doctor (if applicable)
    7. Original Policy Document
    8. Deeds of Assessment
    9. Claim Discharge Form
    10. Nominee’s photo identity proof (like Aadhaar card)
    11. Nominee’s address proof
