You can fill a form online. Go to the insurance company website. Fill in their ULIP online form. Fill in additional details about Sum Assured, Policy Period, and select a plan.

You can also do this offline by getting in touch with an insurance agent.

Along with the filled in form in both online and process and with agent, submit the following documents

Age Proof

Identity Proof Aadhaar Card, Voter ID Card, Passport, etc.

Report of Medical Examination,

Proof of Income bank statement, IT returns,

Address Proof Rent Agreement, Telephone bill, electricity bill, etc,

PAN card

Aadhaar card
