Write an application letter to the bank manager

Take address and ID proof (originals and self attested copies)

Submit the application and the documents

Pay the bank charges (ranging from ₹40- ₹100 depending on the reason you’re asking for a new passbook)

The bank will then give you a new passbook (generally on the same day)
